Basically the Right Hand left hand thing means that two parts of the staff do not communicate.

A doctor will prescribe for you a medicine, and tell you do not drink milk. The nurse will bring you the medicine(most of the time) and a bowl of cereal or some other food with milk in it.

And the whole Caribbean is Third world. The real def of third world state is; a state  that has experienced colonial rule, is democratic and whose economy was developed for the purpose of providing goods to the colonial power.

First world Countries comprised countries like North America and England(countries with free market economies and no recent history of colonial rule)

Second World Countries are those communist states like Russia with planned economies

Third World Countries are those who were just recently under colonial rule, and whose economies where centered on the provision of goods to colonial powers and not towards a development focus.
