Ditto what's in the mouth but I believe you are imagining the third eye only thing I see is either a shadow or another spot behind the ear
Welcome to Weird and wonderful world of "AI Hallucination" Peeps. If you look at many of my Upscaled Posts, I Allow The AI Process to Hallucinate by adding Weird and Wonderful Extras. Yes that's an Eye, and the mouth is a Form of A Cats Outline. I do not choose how or What it Hallucinates But I Do Enjoy Them and Peeps Spotting Them when i post them.

My bad on that part of my reply to you but that area is in front of or above the ear not behind thus I did not look in that area

Example Of Not Just Eyes, its loosely based on the Prompt, This One Is A Vampire, add hallucination upscale, and it shows the "Stolen Souls " Phrase i used in the Original Prompt on the site i use.
The eyes have it!
