How do you create Live Icons?

I would like to alter the live icons of the Icon Packager. I am using the paid for version of Icon packager and have put an image for the front and back in icons. My question is, How do I alter the way windows displays the generated images?

I hope that made sense, I was a little tired when I wrote it. What I want to do is change the the images that are displayed on live icons.
