QoT change form bug

I encountered an odd bug with QoT when changing forms. It was a lagging game so it wasn't very responsive and that may be the only reason it occured.

I was trying to change to closed form to shield myself thinking I was going to die so I hit 4 a few times while it was lagging and she started to change. When she got to closed form both shield and change form were red and didn't change back. I didn't die and was wondering around with no spells to cast. Upon my death and respawn I was in open form again and could cast spells as usual.
