as there are too many programs out there to make an Icon for every single one of them, it would be quite impossible to build a pac to cover them all. Tho some Icon packs are more complete than others and do offer icons for popular 3rd party programs, there will never be a "complete Pack"
There are galleries of individual icons that can be uysed to fill in the "missing" ones.

'everything' and 'all' and pretty inclusive
which version of Publisher, Access, etc? 2002, 2003, etc? in order to properly associate the icon to the right place it helps to have that application installed - not everyone bought Office (or maybe they didn't buy the version you have), not eveyone is running windows defender, etc
I won't even bring up you haven't said if you mean 98, ME, MCE, XP (and version), Vista (and version), Mac, Linux, unix, amiga, etc
So here's an idea: How about not messing with icons that don't match the type described in the package? (i.e. when IconPackager first loads NO changes are made, then when a package is applied the extensions with custom icons get changed, the ones that don't get LEFT ALONE.)
I say this mainly because the icons associated with .dll and .jar (among others, but those were the biggest) got changed to something else for me, and the only way I found to fix them was to reinstall Windows and Java. (thank God for Win7 having the RUI option)
