user picture keeps resetting to chess piece

Basically, every once in awhile my account's picture will change to the default chess piece picture.

Any idea how i can fix this?

just on logons or elsewhere also?

it completely resets my accounts picture.

i have to go into user accounts and pick out a picture for it each time.

then that shouldn't be LS, it just uses whatever your user account picture is
